Patient forms
Print and complete the new patient form and bring it to your first visit. You can also fill it in, save it, and email it back to us.
What to bring to your first visit
A little preparation keeps your first appointment moving. Bring what applies to you. If you are missing something, our staff will sort it out with you at the desk.
A photo ID
A current driver's license or other government-issued photo identification.
Your insurance card
Bring your insurance card. For a workers' compensation or personal-injury case, bring your claim number or case information instead.
Any referral
If your primary care doctor or another provider referred you, bring the referral paperwork with you.
Recent imaging or reports
Any X-rays, MRI, or other reports for your hand or upper-extremity injury, on disc or in print.
A list of your medications
Write down the medications you take, with doses, along with any allergies you have.
Adjuster or attorney contact
For a workers' comp or personal-injury case, bring your claims adjuster's or attorney's name and contact information.
Two things worth handling ahead of time
If you haven't yet, request your appointment and check what your coverage means for your visit.
We'll have your paperwork ready.
Call the office to have your new-patient forms sent, or complete them when you arrive. Request an appointment and our staff will take care of the rest.
